Fingerprinting Process

An FD-258 fingerprint card with the ORI number KY920170Z is used for fingerprinting. The ORI number is the number assigned to the Kentucky Board of Nursing. It lets the fingerprint processing center know where to send results.

Fingerprints may be taken at a law enforcement agency or at a UPS center.
When the card is completed, it should be sent to the Kentucky State Police along with the $16.50 fee. DO NOT send it to the Board of Nursing.

Licensing Process

Once the application, fee, fingerprint and criminal background check results, and supporting documents have been reviewed, the Board will determine if you are eligible to test.

If you are eligible to take the NCLEX, you will receive an authorization to test (ATT) and instructions on scheduling the test.

When passing NCLEX results are received by the board, a license will be issued.

License cards are NOT sent out. Applicants will receive an email containing their license number and expiration date.
