Fees: $143.00 This fee covers the following:

Application for licensure fee: $93.00
Fingerprinting and Criminal Background Check fee: $50.00

All fees must be in US dollars, payable to the Iowa Board of Nursing, and are non-refundable.

Highlights:
Applications for the state of Iowa may be submitted online, or in paper. If using the paper forms, you must print out the form, fill it out, and mail it in to the Iowa Board of Nursing.

Paper applications MUST be completed with black or blue ink.

Submitting the application for licensure by examination does NOT register you for the exam. Applicants must register and pay separately for the exam by going to: www.pearsonvue.com.

The State of Iowa requires that two (2) fingerprint cards be submitted along with the application to license by examination.

The state of Iowa requires that a Waiver for Completing Criminal History Background Checks form be filled out and submitted along with the application.

Important: If you are using the paper application, both fingerprint cards, and the Waiver for Completing Criminal History Background Checks form MUST be sent together with the application form, or the application will be returned.

As part of the application process you must:

Complete the Iowa Application for Exam Form (Online or Paper). Submit the application along with the appropriate fee $143.00 payable to the Iowa Board of Nursing

Submit two (2) completed fingerprint cards along with application. DO NOT FOLD OR BEND THE FINGERPRINT CARDS.

Submit a signed Waiver for Completing Criminal History Background Checks form along with the application (the $50.00 background check fee is included in the $143.00 application fee)

Request official transcripts from your school denoting date of graduation and degree conferred. The transcripts should be sent by your nursing school directly to the Board of Nursing.

Register for the NCLEX. You will not receive authorization to test until this step is completed. The name with which you register must match exactly with the printed name on the identification you present at the test center.

About Temporary Permits

The state of Iowa only issues temporary permits to previously licensed nurses who are seeking endorsement.

Disability Accommodations
To request special exam accommodations due to disability, the following documentation must be submitted to the Iowa Board of Nursing:

A letter from you identifying the nature of your disability and the specific testing accommodation/s you are requesting. You disability must be identified by diagnosis and code number.

Form A found here: Special Accommodations Request Form A This form must be submitted by a medical professional with expertise in your type of disability.

Form B found here: Special Accommodations Request Form B This form must be submitted by your nursing school or program if you required similar accommodations while in school.
