Application for Licensure by Examination

Applicant will need to register with Continental Testing: Apply for RN or LPN License by Examination

Fees

RN/LPN Application Fee: $50.00

Fees are non-refundable and payable to Continental Testing Services, Inc.

Highlights:

  • Application forms for the state of Illinois may be submitted online or on paper.
  • Fingerprints and a criminal background check are required of all applicants.
  • Submitting the application for licensure by examination does NOT register you for the NCLEX. Applicants must register and pay separately for the exam by going to: www.pearsonvue.com

As part of the application process, you must:

  • Register with Pearson Vue to take the NCLEX exam. You should do this before sending in the licensure application.
  • Request that the Director or Dean of your nursing school fill out the Certification of Education form found in the application packet. The form must have the school's seal and must be submitted to the Board along with the application.
  • Make an appointment to have your fingerprints taken. This will begin the criminal background check process.
  • Submit the completed application along with the fee and all supporting documents to the Board of Nursing.

The Fingerprinting Process

Fingerprints must be taken within 60 days from the date that the application is submitted.

Both electronic and ink fingerprints are accepted. In-state applicants should have their fingerprints taken electronically at a LiveScan location. Select a LiveScan location from the list provided in the application packet. When you have your fingerprints taken, ask for a receipt and affix the receipt to your application form.

If you are out of state, you may have ink fingerprints taken, you may do this at any local law enforcement agency or private vendor trained to take ink fingerprints. The IDENTITY VERIFICATION CERTIFYING STATEMENT form must accompany your application.

Licensing Process
Once you are registered for the NCLEX and the application, fee, and supporting documentation have been reviewed, the Board will determine your eligibility to test.

Applicants who are deemed eligible to test will receive an Authorization To Test (ATT) notification directly from the testing service. The ATT letter will include information regarding scheduling the examination.

When passing examination results are received by the Board, a license will be issued.
