RN and LPN Licensure by Examination

Highlights:

  • Currently, online applications for the state of Wisconsin are only available for two local nursing schools (Concordia University Wisconsin and Lakeshore Technical College). Otherwise you should complete the paper application.
  • All 6 pages of the application must be completed.
  • Applicants must indicate what type of degree was received (BSN, ADN, BA, etc.)
  • Submitting the application for licensure by examination does NOT register you for the NCLEX. Applicants must register and pay separately for the exam by going to: www.pearsonvue.com.

 

As part of the application process, you must:

  • Register with Pearson Vue to take the NCLEX exam. You must do this before sending in the licensure application.
  • Submit verification that you graduated from an approved nursing program. This is done by printing DSPS Form #259 found here: Nursing Program Verification Form.
    You must complete and forward this form to your nursing school. The school must send the form directly to the Board of Nursing. Forms received from the applicant will be rejected.
  • Submit the online or paper application for Licensure by Examination along with the fee and any additional forms or appropriate supporting documents.

Licensing Process

Once the application, fee, and supporting documents have been reviewed, the Board will determine if you are eligible to test.

If you are eligible to take the NCLEX, you will receive an authorization to test (ATT) and instructions on scheduling the test.

When passing NCLEX results are received by the board, a license will be issued.

About Temporary Permits:

Temporary permits allow you to work as a graduate nurse (G.N. or G.P.N.) before NCLEX results are available. If you are seeking a temporary permit, you must:

  • Check the "temporary permit" box on the application.
  • Submit the $10.00 fee along with your application.
  • Submit DSPS form # 2433 along with your application.

Temporary permits are good for 90 days. They are also non-renewable. Applicants must take the examination within the 90 days.

If the applicant fails the NCLEX, the temporary permit must be immediately surrendered to the Board of Nursing.

Be aware that you will be required to attach a letter from a qualified professional, and a letter from the nursing education program indicating what modifications were granted by the program.
