RN and LPN Applications: $95.00

Highlights:
- Applications for the state of Pennsylvania may be submitted online or on paper at the links given above. The online application is only for use by Pennsylvania-educated applicants who can use a credit card for payment.
- Criminal background checks are only required if the applicant answers "yes" to question 1 of the Criminal/Disciplinary History questions found on the application.
- Submitting the application for licensure by examination does NOT register you for the NCLEX. Applicants must register and pay separately for the exam by going to: www.pearsonvue.com.
As part of the application process, you must:
- Register with Pearson Vue to take the NCLEX exam. You must do this before sending in the licensure application.
- Take the Nursing Education Form to the Director or Dean of your nursing program. This form serves as verification that you completed an approved nursing program. The school must send the form directly to the Board of Nursing. Forms received from the applicant will be rejected.
- Submit the online or paper application for Licensure by Examination along with the fee(s) and any additional forms or appropriate supporting documents.
Licensing Process
Once the application, fee, and supporting documents have been reviewed, the Board will determine if you are eligible to test.
If you are eligible to take the NCLEX, you will receive an authorization to test (ATT) and instructions on scheduling the test.
When passing NCLEX results are received by the board, a license will be issued.
About Temporary Permits:
A temporary permit allows you to work as a graduate nurse (G.N. or G.P.N.) before NCLEX results are available. If you are seeking a temporary permit, you must:
- Check the "temporary permit" box on the application.
- Submit the $70.00 fee along with your application.
A current and valid temporary permit may be extended, for one (1) year by applying to the Board. The application for extension can be obtained from the Board's web page.
If you fail the NCLEX, you must write "VOID" on the permit and return it to the Board's office immediately. Employment as a graduate nurse must cease immediately upon notification of exam failure.
